Steps for completing required paperwork for OJT classes:

  1. Before a student is scheduled into an OJT class, he/she must complete the Co-op Application, Student/Parent Agreement and Employer's Agreement. and submit to Ms. Young.

    This application will be used by your OJT coordinator to determine if your current job qualifies you to take an OJT course (per Florida state law). No student will be approved for an OJT class without the completed paperwork.

    Keep in mind that you must
    1. Work approximately 5 hours per week for each OJT credit you are receiving.
    2. Work for an employer who carries Workman's Comp Insurance. (You cannot be self-employed.)
    3. Be supervised by a non-family member.

  2. Once your job has been approved, your guidance counselor will schedule you into the course. (If, for some reason, you are scheduled into the course and you have NOT completed the above application, it is YOUR responsibility to complete it immediately so your employment can be verified. Failure to do so, could result in your not receiving credit for the OJT class!)

  3. You are required to keep your time sheet up to date at all times (at least weekly). Time sheets are due twice each quarter (before interims and before report cards). Those dates will be announced on WCAT, and posted in front of the auditorium by the student parking lot (as the date gets close).

    ALL timesheets are to be completed using the attached Excel spreadsheet (NOT handwritten). This sheet automatically calculates all hours AND automatically includes all breaks required by law, depending on your age. Timesheets must be printed on the back of the Employer's Evaluation (commonly known as the "blue form"). This form should be printed on BLUE paper or one can be picked up in Mr. Yancey's office (D135).

    If you do not have access to a computer at home, you may use the computers in A169 before school or after your last class. Please note the lab closes at 2:30 pm each day.

    If you have computer access but do not have Excel, you can complete the form at home, using your FREE Live@EDU account (provided to all MCPS students). Follow instructions provided to access your Live@EDU account.

Forms Required for Co-op:

The Student/Parent Agreement and Employer's Agreement must be completed and signed by the end of the first week of being scheduled into an OJT class.

Time Sheets must be kept weekly and submitted as instructed by OJT coordinator, Mr. Yancey.
